Instructions:
- Enter your name and last 4 of SSN (or 0000 if preferred)
- Enter your available leave balances from your LES on MyPay
- Set the priority order for how leave should be charged
- Enter your leave start and end dates
- Click "Generate Leave Chit" to download the filled OPM 71 form
Important: Sick Leave Restriction
Unless you are Not Fit For Duty (NFFD), you cannot use more than 3 days (24 hours) of sick leave per pay period.
